Public Information Request

The District handles all requests for public information in accordance with Texas Government Code, Chapter 552. To make a public information request, please follow the steps outlined below.

1 – Complete the Public Information Request Form.  If the requested record contains Protected Health Information (PHI), please also complete the HIPAA Authorization Form.

2 – Submit the form(s) to Reeves County Emergency Services District Nos. 1 & 2 by one of the methods below (email is preferred)
